Nurse Case Manager – Complex Community Care (North London / North West London)
Salary: £45,000 + Excellent Benefits

Are you an experienced clinician looking to step into a rewarding leadership role within complex community care? We are seeking a passionate and dynamic Nurse Case Manager to join a high-performing care provider delivering bespoke, person-centred support to adults living with long-term, life-limiting and complex conditions and this comes with a salary of £45,000 and excellent benefits.

This is a fantastic opportunity for a motivated Nurse Case Manager who thrives on autonomy, clinical leadership and building meaningful relationships with clients and their families. Working across North London and North West London, you will play a vital role in shaping care packages that truly change lives.

About the Role – Nurse Case Manager
As a Nurse Case Manager, you will lead and oversee a caseload of adult clients requiring complex care in the community. You’ll design personalised care plans, support and clinically upskill care teams, and ensure safe, high-quality, and compassionate care delivery. You will collaborate closely with CHC teams, commissioners, MDT professionals and families to deliver holistic, outcome-focused care.

Key Responsibilities
Develop, implement and evaluate tailored care plans
Deliver clinical oversight, supervision and competency training to care teams
Support clients with complex needs including ventilation, tracheostomy care, PEG and NG feeds, spinal injuries and neurological conditions
Monitor, assess and review clients’ physical and emotional wellbeing
Ensure safe medication management and clinical interventions
Build strong, professional relationships with families, CHC nurses and commissioners
Contribute to service improvement and uphold high compliance standardsCriteria for Success
Registered Nurse (RGN) with active NMC PIN
Clinical background in complex community care or acute complex care
Recent experience in tracheostomy and ventilator care
Empathetic leadership style with experience supporting HCAs or support workers
Excellent communication and clinical assessment skills
Suitable for Band 5 or Band 6 nurses ready to progress into a leadership role
Full UK driving licenseBenefits Package
Salary up to £45,000 per year (DOE)
Hybrid working: 1 day WFH, 3 days in the community, 1 day at head office (Wimbledon)
Monday–Friday, 9am–5pm
33 days annual leave
