Description :
Position Summary: Collects, displays, and monitors patient care data. Provides information to internal and external customers. Manages patient safety reports. Provides support for continuous quality improvement, occurrence management and confidential quality assurance activities. Key Responsibilities – Utilizes nursing clinical knowledge in the review and abstraction of clinical data concerning a broad range of patient cohorts and clinical conditions. Observes patterns in clinical practice as reflected in documentation, and makes recommendations for practice changes to enhance the quality of care provided. – Reviews patient care data for DHMC committees, quality improvement initiatives, quality assurance and patient safety efforts and both internal and external data requirements. – Assists in the development of criteria for measuring quality. Coordinates data collection and compilation activities as appropriate. – Manages patient safety reports using a web-based occurrence reporting and management tool. – Resolves identified problems both at case level and as a participant in a group to improve processes and/or communication. – Designs formats for data collection. Interprets, documents, and displays data in an appropriate format (graphical, statistical, and/or written summary). – Communicates with clinical staff concerning documentation and facilitates resolution of appropriateness and quality concerns as needed. – Conducts special statistical studies on request. Compiles monthly and annual summary reports and pattern analyses summaries as required. Performs other duties as required or assigned. Minimum Qualifications: – Graduate from an accredited Nursing Program required. – Bachelor’s degree (or matriculation in a Bachelor’s program) required. – 8 years of experience of clinical practice required. – Quality assurance, continuous improvement and data collection experience desirable. – Knowledge of computer technology highly preferred. Required Licensure/Certification Skills: – Licensed Registered Nurse in New Hampshire required.
