Job Description

Employment Type: Full time Shift: Day Shift Description: Under direct supervision of HME Manager, assists in communicating agency services to health care providers, business and community organizations. Using assessment skills, coordinates referrals and inquiries for regulated, extended service, and HME services between staff and referral sources. Adheres to the principles of Service Excellence Guiding Behaviors and the philosophy and mission of Mercy in all aspects of job performance. ESSENTIAL JOB FUNCTIONAL COMPETENCIES: Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with Mercy standards. Coordinates agency inquiries, referrals and physician orders. Is responsible for the triage of medically related incoming calls/referrals with appropriate follow up and coordination with all services lines involved. Assists in establishing and promoting ongoing communication regarding services available that may meet client/family needs to callers, visitors, physicians, and other referral sources. Is responsible to evaluate patients referred for home care to determine appropriate use. Accepts or rejects patients based on admission criteria. Recommends alternative plans of care for patients deemed not appropriate. Assists in the development of an education plan which defines the target goal and time frames based on analysis of referral information and home care industry trends. Provides coordination of care for all clients admitted to the agency by coordinating with facility/hospital discharge planners, physicians, clients/families, and the agency. Establishes a definitive home care plan including appropriateness of the requested services medical supplies and appliances based on the goals mutually acceptable to the patient and their family. Interprets the financial aspects of coverage for homecare services to clients/families. Participates in the intake workflow process and makes improvements to streamline and increase efficiencies. Coordinates initial and continuing insurance verification for services when applicable. Coordinates the documentation and processing of all paperwork related to referrals and inquires accurately and on a timely basis. Assists in the orientation new and existing staff and appropriate to referral process. Follows through the phone calls, i.e., communication with staff, documentation of physician orders, patient/family instructions, or coordination of services. Reports identified problems to manager. Is able to develop and sustain rapport with coworkers. Is able to adjust work in response to change in priorities. Participates in interdisciplinary case conferences to ensure continuity of care and effective utilization of services. Facilitates communication among team members, clients, physicians and other health care providers to assure coordination of services. Participates in quality improvement activities as requested. Notifies hospitals of readmission and provides current information regarding client status. Assures customer service response to patients/families, physicians, and other health care providers meets expectations including promptness, responsiveness and follow through to meet customer needs. Uses a professional approach with patients, families, branch staff, physicians and other personnel. Is knowledgeable of and actively participates in improving patient safety and reducing risk to patients. Complies with Safety and Infection Control Policies and Procedures. Knows, understands, incorporates, and demonstrates the Mercy and Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Mercy and Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior. Maintains confidentiality of information pertaining to clients, physicians, employees, and Mercy business. Demonstrates general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.) Performs other duties and responsibilities as assigned. MINIMUM QUALIFICATIONS: Requires High School Degree. Excellent communication and interpersonal relationship skills. Excellent organizational and time management skills. Knowledgeable of standards and coverage criteria for acceptance into home care in regards to third-party billing preferred. Must have a general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.) Must be comfortable operating in a collaborative, shared leadership environment.
