The Lead Med Certified Personal CareAide is responsible of day to day care of resident which includes ADLS, IADLS, light housekeeping, and assisting residents with medication abiding to Personal Care Regulations. In addition, this Med Certified Personal CareAide Team Leader acts as a mentor to all Personal Care Aides (PCA) and Med Certified Personal Care Aides (PCA).

Lead Med Certified Personal CareAideessential duties and responsiblities include, but are not limited to:

Provide leadership to PCAs and provides department orientation to newly hired PCAs.
*
Assist Personal Care Director with shift schedules and bi-weekly payroll. Prepare a daily work assignment for each PCAs in the absence of supervisor and communicate pertinent information relating to resident care. Daily use of chart to chart report to communicate routine reporting to the PCA who is coming on duty.
*
Assist with New Resident Admission process. Ability to follow facility admission procedures and communicate resident admission to responsible party. Assist with completion of Wellness Assessments, Support Plans, Admission Documents. Provides for orientation of newly admitted residents Participate in resident care meetings.
*
Perform resident assessments. Demonstrate competence in vital signs. Demonstrate competency in the use of weight scales equipment. Document on residents personal care record and in personal care communication log. Record resident care provided and residents response or lack of response to the care..
*
Maintains effective working relationships and open communicationwith residents, family, and physician. Addresses concerns regarding care and initiate corrective action if indicated.
*
Ability to consistently work cooperatively with residents, families, co-workers, physicians, consultant personnel, community agencies, and ancillary service providers.
*
Conducts service reviews to ensure that services are being delivers to residents properly and as prescribed Communicates information regarding services to new residents, changes in services to existing residents.
*
Identifies a change in a residents condition and promptly report changes in a residents condition to charge nurse immediately (i.e. any medical emergency, behavioral changes, bruising or wounds, and certain time reporting which includes sugar levels, blood pressure). Report any actual or suspected physical, verbal, sexual, or emotional abuse of resident or employee.
* Assist residents with daily personal hygiene needs such as bathing, peri-care, oral hygiene, shampooing, grooming, shaving, dressing and undressing, and care of clothing. Assist a resident with incontinent care. Ordering of resident incontinent supplies & department supplies. Assist a resident with colostomy care.
*
Assist with tasks of daily living such as securing transportation, making and keeping appointments, use of telephone, bed-making, and light housekeeping. Assist residents with mail delivery, opening and reading, as requested. Assist a resident with basic meal preparation. Maintains resident room and belongings in a neat and orderly fashion, emptying wastebaskets and washing dishes in resident apartment. Does resident laundry gathering, washing, folding and putting away clean clothes. Maintains proper storage and handling of linen and supplies
* Assist a resident with ambulation. Assist residents with transfers, to and from bed, to and from chair, to and from toilet. Safely transport residents to/from activities, meals, church services, hairdresser, etc
*
Encourages, engages and participates in therapeutic activity programs
*
Comprehends and practices approved asepsis and infection control procedures
*
Has working knowledge of all policies and procedures pertaining to the Personal Care Department
*
Has working knowledge and is able to perform PCA positions and the PCAL position.
*
Interacts daily with Personal Care Director, offering advice and suggestions regarding direct resident services
*
Ability to recognize an emergency situation and initiate immediate help. Identifies safety and or maintenance concerns, i.e. broken equipment, fall risks, etc.
*
Responds to all call emergency bells
*
Is a qualified Practicum observer to review Med Tech Training.
*
Demonstrate the Five Rights of giving medication Corresponds with pharmacy as needed
*
Proper storage of medication in cart locked clean, and organized
*
Demonstrate proper documentation on MAR including missed dose, PRN medication and discontinued medication. Daily counting of narcotics count every shift with PCA who is relieving you.
*
Administer and document medications and treatments per physician orders. Medication including OTC medication must have a physician order if not, medication must be removed and reported to the charge nurse. Assist a resident with self-administration of medications, medication reminders, and obtaining a medication refill through pharmacy.
*
Demonstrate for residents how to use record glucose testing prescribed by the physician and the use of inhalers, mini nebulizers. Process medication refills as necessary.
*
Reporting medication errors

RELATED EXPERIENCE: Three years experience as a Med Certified Personal Care Aide

EDUCATION OR DEGREE REQUIREMENTS: Completion of high school diploma or GED

LICENSES, CERTIFICATIONS, REGISTRATIONS: Medication administration certificate issued by DPW; Diabetes Education Certificate
