We have an exciting opportunity for a Senior Planned Care Nurse to join our Primary Care Team at HMP Altcourse, a category B prison located in the Fazakerley area of Liverpool. This is a very busy remand prison with a high turnover of patients.
Within this role, you’ll be an integral part of the healthcare team. Within this role, you will work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions.
Hours
We operate a 7 days a week Service. You will be expected to work approximately 1 in 3 weekends and some nights.
Shift Times:
7am – 8pm
8am – 9pm
9am – 10pm
7.45pm – 7.15am
Salary
This position is full time, you’ll receive an annual salary from £43,043 to £50,256 per annum – depending on experience.

Candidates are required to have a Legal right to work in the UK on a permanent basis and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
What you'll be doing
As a Senior Planned Care Nurse, your responsibilities will vary. You will:
Provide visible, effective leadership and act as a clinical role model
Support staff with complex clinical decisions
Promote a culture of safety, learning, and continuous improvement.
Carry out planned Care Management
Lead and coordinate planned care programmes (e.g. vaccination, screening, health promotion).
Oversee management of long-term conditions (diabetes, asthma, hypertension, COPD).
Line manage and support the Practice Nurse in delivering preventative and planned care
Monitor and audit planned care pathways to ensure clinical governance compliance.
Rota and Workforce Management

Qualifications required:
Registered nurse – Adult
Current professional qualification (NMC)
Evidence of Continued Professional Development
Understanding of the complexities of multi-disciplinary and multi-agency working
Excellent communication skills
Proactive problem solving skills
Motivated as an individual and when working in a teamAlthough not essential ideal applicants would have experience in a hospital, community or prison environment.
